Nurse Practitioner - 3-4 10 hour days a week

DescriptionJob Summary:The Nurse Practitioner (NP) will help implement a new care delivery model focused on a differentiated patient experience and provide comprehensive primary care services at the Health Center. In this capacity, the NP is responsible for independently providing direct patient care and being part of a comprehensive primary care team. The scope of practice includes all aspects of patient care, including examination, diagnosis, treatments, testing, and consultations for adult and adolescent patients above the age of 12 in an advance primary care medical home setting. They may also conduct physicals, order tests and serve as a patient's primary healthcare provider. NPs are also able to prescribe medications.Job Duties

• Provide patient-centered, comprehensive primary care services consistent with the patient centered medical home care delivery model.
• Participate in the selection of suitable treatment plans for all patients.
• Assist other care providers and extended care teams to comply with the Health Center’s patient care practices, policies, and procedures.
• Participate in the selection of cost-effective treatment modalities.
• Provide continuous availability for medical and technical questions to the employees, patient care staff, and ancillary care providers including after hours coverage.
• Collaborate with the management team as needed to ensure improvement of patient care processes, policies, and procedures.
• Cooperate with clinical outcome reviews and measurements standards as implemented.
• Other duties as assigned.

Education And Experience Requirements

• The Nurse Practitioner is required to be a board-certified, Washington-licensed NP with a minimum of 5-7 years of professional clinical experience in primary care practice. Strong preference for experience in an integrated, multi-disciplinary medical home model.
• Proficient in promoting the Health Center and comfortable in public speaking and education settings including the development of topical presentations.
• Current BLS (Basic Life Support) certification.

Physical Requirements

• Physical ability to execute job responsibilities that may include: Standing or stooping for long periods of time, Mild physical labor to complete the tasks at hand, Exposure to blood, chemicals, and other bodily fluids.
